Please let me know if I missed anything or if there are any corrections.
Equipment
Please note that you must identify an item for it to count, simply having it in your inventory will not work. Throughout the whole journal, using scrolls of identify or divination and the Examine button are strongly encouraged.
In order:
Worn Shortsword, Mage’s Staff, Dagger, Studded Gloves, Rapier
These are starting weapons for each of the five classes, so once you’ve unlocked everyone, you’ve got them.
Shortsword, Handaxe, Spear, Quarterstaff, Dirk, Sickle, Pickaxe
These tier-2 weapons will be found mostly early on in the dungeon, except the Pickaxe, which you will get from doing the Blacksmith’s quest on floors 11-14.
Sword, Mace, Scimitar, Round Shield, Sai, Whip
Tier-3, also found randomly throughout the dungeon
Longsword, Battle Axe, Flail, Runic Blade, Assassin’s Blade, Crossbow, Katana
Tier-4, also found randomly throughout the dungeon, usually later on
Greatsword, War Hammer, Glaive, Greataxe, Greatshield, Stone Gauntlet, War Scythe
Tier-5, same as above, more rare than the others.
You may find scrolls of transmutation useful for obtaining these if you’re not finding them at random, but they will not change an item to one in a different tier.
Cloth Armor: If you’ve started the game you have this. Congrats!
Leather Armor: Tier-2, found liberally throughout the early floors
Mail Armor: Tier-3, also found easily
Scale Armor: Tier-4, slightly more rare
Plate Armor: Tier-5, the rarest
The remaining five are the end-game armor for each of the five classes in order. You receive these by defeating the Dwarf King and wearing his crown. They are: (Hero’s) Platemail / Robe / Garb / Cloak / Breastplate
Oh boy, here we go. In order:

Curses: Annoying, Displacing, Dazzling, Explosive, Sacrificial, Wayward, Polarized, Friendly
You must identify enchanted or cursed items for them to be added to your journal. You can find them randomly throughout the dungeon or by using Stones of Enchantment, Scrolls of Enchantment, and Curse or Magical Infusions.
Glyphs: Obfuscation, Swiftness, Viscosity, Potential, Brimstone, Stone, Entanglement, Repulsion, Camouflage, Flow, Affection, Anti-Magic, Thorns
Curses: Anti-entropy, Corrosion, Displacement, Metabolism, Multiplicity, Stench, Overgrowth, Bulk
All the same methods as above apply for obtaining these, in addition to the Arcane Stylus.
