Due to the nature of this type of guide, there may be spoilers. They will be marked accordingly.
At this time, there is no way to verify whether the total of items mentioned here encompasses all in the game. I did my best to scrape every corner, but there’s no doubt I missed a Life Fragment or Graceful Essence somewhere.
Tutorial Level (1/2)
After a short cutscene, you will begin your adventures as the Wanderer. You awaken in a glowing blue pool with no items, no weapon, and no memory of how you got there. Walk out of the room and up the ramp.
You’ll come across a door on the right side. This room contains the first Memorem that we’ll come back to later.
Continue straight and roll under the wall at the end. You’ll see a white creature sitting on the ledge of a giant, ghostly statue. Impossible to miss. Get closer and talk to it.
To the right of the statue on a balcony is your first item. Collect it to receive 1x Life Fragment. You can use these in a pinch to restore a small amount of HP. Turn around and go back towards the statue, turning right. Ahead, you’ll see a crumbling hole in the wall and two ways to go.
Left will bring you to the corpse of a Wanderer and two items, 1x Wanderer’s Mask and 1x Memory Shard. Both are necessary for achievements.
Right takes you to a few more items and your first weapon, easily missable. Jump up to collect 1x Life Fragment.
Jump up another level and in the corner of that platform is the pillar we’ll come back to later after we obtain all Memory Shards to get the cool weapon. Look right and there is another platform opposite to where you are. Double jump and air dash across to get 1x Life Fragment.
In front of you in the left corner will be a pillar similar to the last one which holds the beginner weapon: Catharsis, Limbo Blade. It will automatically be equipped. Hop down behind you and head towards the crumbling hole in the wall. You can now attack it to open the path to continue.
Inside will be two blocks on the left and right, both with items. The left contains 1x Delicate Essence. You crack these open to obtain a small amount of essence, used in leveling both your character and your weapon. The right has 1x Life Fragment and 1x Delicate Essence. Your first Limbo Well will be in the center. They can be identified as the black pillars with a green orb on top and shadow pouring vertically from them. This is where you can heal to full and will revive from after death, but it also resets the area’s enemies. Activate it.
On the left of the well is a door by a brazier, but it’s currently blocked off. Your only path (for now) is up the two alternating platforms in front of you.
Once you reach the top, talk to Y’aak again and collect the item inside the display, 1x Ancient Catalyst. This acts as your healing pots. It starts with 1 charge (1 heal) and can be upgraded throughout the game. It is recommended to equip it in your first slot of Quick Access, which will appear in your item wheel in the bottom left of the screen.
Look left and you’ll see another crumbling hole in the wall. Attack it and open the chest inside to receive 1x Delicate Essence, 1x Life Fragment, and 1x Ether Fragment. Like Life Fragments, they restore Ether (mana) instead of HP. Head back out. You’ll see a lever with a purple orb on the handle beside the gate. Activate it and go through.
Just outside the gate is a teleport warp connected to an area we’ll access later. It’s unactivated for now. This is how we’ll return for the weapon and Memorem.
Look right and you’ll see 2 enemies. They are not particularly hard and can be vanquished in a few light attacks. There is also a bridge on the left that we’ll unlock later at the end of the tutorial.
After felling the enemy at the end, turn back and go through the lit doorway on the right. There is 1x Residual Essence and an enemy lurking behind. The door inside of that hallway on the left is another one currently blocked off. Continue through and up.
On the left will be a statue that triggers a vision if you get close enough.
Keep going down the hall and up the ramp to find 1x Residual Essence. Up the stairs on the right only leads to a double-door shortcut you won’t open until the end. Head back in the direction where you came from by going through the hallway again and down the ramps/stairs and out the lit doorway.
Across from you is a gate locked by chains and a ramp. Go up it and turn left.
Tutorial Level (2/2)
Turn around and there will be a chest. Open it to receive 1x Stabilized Essence, 1x Life Fragment, and 1x Ether Fragment.
Behind you is the gate. Attack the chains to permanently open it.
Go out the gate and turn around to go back up the ramp, heading to the right this time. A friendly Wanderer will be standing there. Talk to her to receive your first Artefact, Nether Bolt. It can be equipped in the weapon menu. Past R’lya is another lit doorway with an elevator inside. Simply stand on it to ride it to the bottom floor.
There will be 1x Residual Essence and the blocked door leading back to the Limbo Well. Attack to open it. Get back on the elevator.
Get out of the room and go up the stairs in front of you. Beware of the Gunner hiding in the pillars when you go right. Enemies will sometimes drop stone materials, either Beryl, Eibonite, or Crystal. These are used for crafting items later on.
Go up the steps behind where the Gunner was and turn around. There is a small parkour challenge that will award you with your first Eibon Fragment and 1x Delicate Essence. Start on the right side (shorter blocks) and double jump + air dash onto the first one. Then single jump along to the end. When you reach the last of them, double jump and air dash onto the first tall block on your left. You cannot make the jump onto the ledge from the shorter blocks.
From that large block, look right. Use the extra height to double jump (and air dash if short) onto the ledge and get the fragment. Get back onto the same large block you used.
Look left. Jump onto the rooftop and walk straight to the end. There will be a window with broken metal where you can enter.
Jump down and you will see 2 items on the glass floor, 1x Superior Essence and 1x Hadronite, used for weapon upgrades. Go back up and out the broken window to the same large block. Turn around to the left and single jump along until you land on the one before the item. Do not jump for it blindly as there is nowhere to land and you will slide off its hitbox. When you are ready, jump for it and spam the collect button/key. You should get it as you’re falling down back to where the Gunner was.
Head up those stairs and to the right. Across the bridge is two ways to go. On the left (pictured) is an arena surrounded by orange braziers and dead Wanderers with an elite enemy in the center. It will reward 467 essence if defeated and it will not respawn. It is large and therefore slow, easily felled if you stay to the side or behind it doing 2 hits at a time. In the corner behind where it was initially standing is 1x Graceful Essence.
You can now turn around and leave the arena to go to the right side. There will be a small alleyway with some lost souls. They only reward 15 essence and are easy to run past, but may make things difficult if the shielded enemy from within the doorway on the left is pulled outside. It’s best to just put them out of their misery before entering. Be wary of the Gunner on the top level in the room.
Go up the stairs and into the next room. There is another basic enemy to defeat and a sideroom straight ahead with some elevators. Go down the one on the right first.
This will take you to the blocked door that’s in the hallway with the statue that gave you a vision. Attack the boards to open it, then get back on the elevator. Get on the left elevator now to go to the next level. There are two enemies standing guard in front of the next elevator to take at the back of the room. Defeat them and ride that elevator to the most top level. Another ghostly statue will be there. Exit through the doorway on the left (or behind the statue if you’re facing the same way as pictured.)
You’ll reach a platform where the game will tell you to jump off. When you land, you’ll see two levers. They each lower a half of the bridge aforementioned. Activate them and turn around.
In front of you is a Matrix-like hallway. Go down it and break open the door. This is the double-door shortcut from earlier. Go out it and jump down. If you turn around, you’ll be at the bridge. In between the bridge halves will be 1x Residual Essence.
Heart of Limbo
The other one is tucked in this little cubby. Look left before going through the arc.
As you continue up the stairs, Y’aak will stop you and talk briefly before you enter the new area. A lore cutscene will trigger. Upon watching it, you will be granted:

First, go up the steps to your right. This is where you’ll level up your character using essence. If you defeated all of the enemies without dying, you’ll have 1,206. After cracking open all of the essence items collected, you’ll end up with 4,506. This will get you 7 levels with some leftover.
Walk to the left. Down the stairs, you’ll see an item. Collect it to get 1x Catalyst Charge.
Keep going forward, ignoring the path on the right going down to a machine for now (it’s locked). Cross the bridge and you’ll see R’lya on the left. Speak to her first. On the right is Kûta. They are a bladesmith that gives you a hint on how to repair the Crucible in the middle. You can then use the Catalyst Charge you just picked up to increase the times your Ancient Catalyst can heal to 2.
Head back to the center and talk to Y’aak. It will always sit on this rock when you come back to Limbo, offering hints and information on bosses. The first time you speak with it here will reward you with the Mark of the Whisperer. This item allows you to teleport back to the last Limbo Well you touched at any time. It has unlimited uses and does not consume anything.
Exhaust its dialogue. Eventually, you will reach this menu. Do not use any Eibon Fragments for leveling up. This will lock you out of an achievement. Leave.
You can now activate the Limbo Well and talk to Choronzon, the giant man with a key behind it. Y’aak mentions this to be Hypnos’ royal guard in the cutscene. He gives you the Mark of Choronzon, which allows you to teleport back to the Heart of Limbo in the same way that the Mark of the Whisperer does for Limbo Wells. I would recommend bounding both of them to Quick Access.
Activate this big gate. It will allow you to travel between the different regions quickly. The place that shows is our next location, but there is still more to do here so exit out of it.
Behind that is a locked gate. You can only get access to this area after obtaining the key from Choronzon. However, it is locked to a specific ending (mentioned later).
On the opposite side of Choronzon is a broken teleport gate, and behind that, a small subarea. Go across the broken path, careful not to fall through the gaps.
On the left is a teleport warp that will take you back to the tutorial area. It will activate as you get close to it. Go ahead and pass through it to the other side. This will activate that one as well.
You probably saw it as you flew by, but there is a secret cave underneath the warp. Jump down onto the walkway and into the cave.
There is 1x Stabilized Essence and 1x Hadronite. Examine the data pillar. Leave the cave and back up to the warp. Turn to the right and head into the gate we opened near the start. Go all the way back to the beginning of the game, past the ghostly statue and roll under the stone. Head down the ramp and into the sideroom on the left. We are going to unlock the Memorem now that we have an Eibon Fragment.
This room in case you forgot.
Interact with this rock. It will consume an Eibon Fragment and unlock the Memorem of a Logician Priest. Read it, and you are now safe to run back and warp to the Heart again.
Keep walking to the left and in the center of three red shrines is this purple object, the physical Heart of Limbo. Remove it to receive:

This will cause your save to close and you’ll be sent back to the home menu. Hit Continue and you’ll reload in where you left off. Don’t worry, this won’t affect your save or endings in any way. It’s more of a joke than anything.
